Recently, from the US Department of Transportation Highway and Transportation Safety Administration (NHTSA), in every accident reviewed by ODI, there is evidence that the accident was caused by the wrong use of the pedal.

According to the investigation report from the US Highway Safety Administration (NHTSA), the investigation has been filed on January 13, 2020 and closed on January 8, 2021. It lasted one year and the vehicle models covered the Tex from 2012 to 2020. There are four models of Model S/X/3/Y produced by the company. A total of 246 accidents were investigated. The report stated that after reviewing all the data, ODI found no evidence to support the vehicle's SUA defect or brake failure. In every accident reviewed by ODI, there is evidence that the accident was caused by the wrong use of the pedal.

In addition, no reasons for failures were found in the accelerator pedal, motor control system, and braking system, including the reasons for the accelerator failure caused by unreasonable design. The report also pointed out that the accident was caused by software, malfunctions, etc. The theory caused by the failure of the electronic system is an inaccurate assumption. And failures caused by system failures caused by electronic systems such as software and bugs have also become important problems.
